9.2.1 Submission of the Complaint

Complaints of an academic nature should be submitted in writing to the Academic Advisor. Complaints of a clinical nature should be submitted in writing to the Program Director.  In addition to these methods of complaint submissions, students are able to use an intradepartmental online Incident Report Form which will be received by the CSD Program Manager via email.

There are two avenues for reporting incidents. You have the option of choosing whether you would like us to formally investigate your report, follow up with you personally throughout the process, and take action within the department (Formal Incident Report, or FIR); or simply to notify senior administration within the department and file an informational report with no recommended follow-up (Speak Up Moment, or SUM). 

The process is designed to protect students’ anonymity by having the forms sent directly to the Program Manager in the Communication Sciences & Disorders Department. You will need to provide your name regardless of the type of report you are filing, but your name will stay confidential unless you have requested a formal follow-up (FIR) within the department, at which point the Program Manager will inform the CAS Program Director, CSD Chair and Associate Chair of your name and the contents of the report.
