Immunization (State of Illinois)
The Illinois Department of Public Health has finalized the immunization requirements for all postsecondary educational institutions.
Students born before January 1, 1957, may be required to submit a medical history and information form (provided by Student Affairs), or to provide Student Affairs with proof of birth (i.e., birth certificate, driver’s License, or personal identification card issued by the Secretary of State) for exemption. Student born on after January 1, 1957, may be required to submit to Student Affairs proof of immunization to measles, mumps, rubella, tetanus, and diphtheria.
