Program Manual Attestation

 

The students in the Entry Level OTD Program are responsible for reading and understanding the policies and procedures that reflect the purposes and requirements of the academic and fieldwork programs at the MGH Institute of Health Professions, as provided in the Institute Catalog and the OTD Program Manual.

The statements contained within the OTD Program Manual serve as a supplement to the Institute Catalog, delineating policies and procedures that are specific to the Entry Level OTD Program.

The OTD Program Manual has four parts: Part I contains the Program Philosophy and Curriculum, Part II contains the Technical Standards, Part III contains Academic Policies and Procedures, Part IV contains Fieldwork Education Policies and Procedures, and Part V contains information about the Advanced Doctoral Experience.  Additional information and resources can be found in the class D2L course, fieldwork D2L course, and in individual courses.

Students will be expected to attest to knowing and agree to abide by the policies and procedures contained within the Institute Catalog and the Program Manual for the Entry Level OTD Program.  Any questions regarding policies and procedures should be directed to the student’s advisor, Department Chair, Program Director, or Director of Clinical Education.

 

I understand that I am responsible for knowing and abiding by the policies and procedures contained within the Institute Catalog and the Program Manual for the Entry Level OTD Program.  All questions have been answered to my satisfaction. I understand these policies are subject to revision, and that I will receive due notice of any changes that are relevant to my status in the program.
