8.2 Format of Complaints

The complaint should describe the issue in detail with a specific and complete explanation of the circumstances and any supporting documentation. The written complaint is filed in the CSD Program Office and is housed in a designated “Complaint” file. It should also specify the changes that are sought by the complainant.
